Something was missing. Two somethings, actually.

The blood pressure medications the member was taking last year were not among her other
medications. They are at the pharmacy, she said. They have been there for the past three weeks.
She said she couldnt afford the gas to pick them up and she didnt have any family or friends living
in town who could help.

No surprise. Her blood pressure was 210/100.

I contacted her primary care provider who recommended she be taken to the hospital. She refused.

With the pharmacy being a few minutes away, I went to pick up the medications for her. However,
the prescriptions she needed were not the ones they had ready. I called her PCP again and the nurse
immediately called in the missing prescriptions. As I waited, I also bought her a pill organizer so she
could be sure of taking her medications every day. Twenty minutes later, I was on my way back.

At the end of the day, on my way home, I stopped by to recheck her blood pressure, which was now
170/80. The member was very appreciative and hugged me as I was leaving.

I left a message with her PCP office updating everything and requesting they call her within the
week to schedule a blood pressure check. A care management referral was also made.
