In order for information to flow from the Pre-Visit Care Coordination document to the Providers
document, the Care Ambassador must:
USE THE CORRECT DOCUMENT TEMPLATE
AND
DOCUMENT IN THE SAME PATIENT ACCOUNT THE PROVIDER WILL USE
VERIFY that your PHONE appointment and the Provider visit are booked on the
same account before opening your document template.
