the certification as appearing on the approved plan Psd 05 039289 as surveyed for Erlinda
M. Santos, et. Al. covering Lot 2190, Cad 296, Buhi Camarines Sur.

Please be informed that as per OCT No. RP 10450 ( 86 50) of the subject lot, the
patentee should be Hermogenes Malazarte onstead of Hermogenes Malasarte, hence, this
humble request be granted.
