We would like to inform you that your son/daughter, ____________________________________, has been
chosen to be part of the SHS Mananayaw ng Kulturang Pilipino contestant on the Say Dayew Folkdance Competition
this coming Febraury 23, 2017 in celebration of the Dagupan City Childrens Congress. This will showcase every
youths skills and talent.

In this regard, we ask for your patience and cooperation by allowing your son/daughter to attend practices
during weekdays (Monday to Friday) from 4:00PM to 7:00PM and during weekends (Saturday and Sunday) from
8:00AM to 4:00PM (packed lunch).

PARENTS CONSENT

I am allowing my son/daughter, __________________________________________, to join the above-

mentioned activities and competition. I am fully aware that when any untoward incidents happened during the said
practices/competition which is beyond the scope of the school then Bonuan Boquig National High School will not be
liable.

This is, also, to confirm my childs attendance in all practices most especially on the contest proper.
