Remedial Action / Work done “Tbe no 5 and its adjacent Two tubes (on each sides) cut & require size spool pieces welded .WSB-39 vicinity tubes found {eroded Thickness taken of 18 tubes (9 each from both sides) & severely eroded tubes (thickness <20%) was replaced. | Tota-28 joints 7. Root Cause Analysis: In interna inspection Burner panel tube no-S commer -2has fish mouth opening (thick lip rupture), This type of leakage may cause due to Abnormal firing rate lame impingement .This isa fist time fallure ia ths comer since commissioning. Also burner tip got eroded & profile changes ofall corners, must be replaced in AOH '. Preventive action suggested (Short Term): ‘Avoid flame impingement caused by fireball shifting due to any one burner out of service 9.
