Employee Assistance Program (EAP)
Southwest Conservation Corps is offering to its employees and their houschold members an Employee
‘Assistance Program (EAP). This benefit provides a free confidential service to employees whose
personal issues are affecting their work performance, family, or other aspects of their lives. MINES is
providing these EAP services.
A personal issue may be any situation or condition that interferes with your quality of life or your level of
coping. Issues may include chemical dependency, emotional difficulties, family, marital, parent-child or
other stressful conditions.
‘The EAP is designed to assist you in obtaining the correct professional help for your concems. When you
contact the EAP, a staff member will schedule an appointment for you with a therapist to assess your
situation, advise you of alternatives for help or provide short term counseling, if appropriate, in the 1-3
sessions available.
Participation in the EAP will not jeopardize your job security nor will it affect future promotional
opportunities.
If your supervisor refers you for a work performance problem, the EAP staff member will only let him or
her know if you have kept your appointment, and whether you have agreed to accept the help that was
offered. No information concerning the nature of the problem will be released without your written
consent.
‘The EAP benefit also includes access to the Mines and Associates Work Life web site where you will find
thousands of resource articles, self-search locators, interactive on-line trainings & wellness assessments.

Southwest Conservation Corps assumes the cost for the EAP. Other costs, for example, treatment beyond
the EAP sessions, may be covered in part or in full by your insurance plan. It is your responsibility to
verify your benefit coverage.
‘You can contact the EAP directly at 1-800-873-7138. Appointments will be made at mutually convenient
times. The telephone number is staffed 24 hours a day and seven days a week.
CONFIDENTIALITY is one of the most important aspects ofthe program. If you contact the EAP
directly, no one in the organization will know about it - unless you tell them.
