This is an official written reprimand for the failure of not being able to (state infraction). Such is a violation
of SOP, specifically policies guiding the work-from-home benefit. It is also construed as insubordination.
You have received verbal counseling for the earlier offense in which you (state again the
offense/infraction). With this letter of reprimand, I am reminding you of (explain importance in following
policiess). Because of your position, further diligence in carrying out your responsibilities is required .
Another breach in our confidence in your ability to carry out any of your expected role will result in
additional disciplinary action up to and including the possibility of employment termination.

Dear Ms. Nepomuceno,
This is a first written reprimand being issued to you for violating the university policy on
tardiness, as outlined in our Human Resources Manual.
In our manual, PEN expects all employees to be at their work area during the designated times.
All employees must comply with work schedules. Reporting for work after-even one minute
after-the official time shall be considered as tardiness.
Any future violations of our policy on tardiness, or any policy, may result in additional
disciplinary action up to and including possible termination.
