The approved OWTS design is for a 38,410 gallons per day treatment train consisting of the following: 3 - 25,000 gallon Septic Tanks 1+ 25,000 gallon Equalization Tank / Anoxic Tank 8- Orenco Advantex AX 100 Treatment Pods 1 - 30,000 gallon Recirculation Tank 1 + Orenco MBBRd-42 Custom Moving Bed Biofilm Reactor 1- 15,000 gallon Dosing tank toa Pere Rite drip dispersal System. ‘An enhanced treatment system is proposed in order to mitigate against possible adverse impacts of residual onsite wastewater pollutants to groundwater and surface water quality. Pursuant to the application approval, the following stipulations shall apply and shall be considered an integral part of the approval: 1. The following discharge limits as measured at the 15,000 gallon Dosing Tank in the treatment train on the plans shall apply: Biochemical Oxygen Demand (Monthly Average) 30 mg/L. ‘Total Suspended Solids (Monthly Average) 30 mg/L G0 pussPage 2 Total Nitrogen 10 mg/L Wastewater flow shall not exceed a monthly average daily flow of 38,410 gallons per day. Wastewater flow shall be measured monthly so as to ensure compliance with this stipulation, ‘The permit issued to the above-named persorvapplicant hercinafter referred to as the “permittee,” only. The permit may be automatically transferred by the peimittee to @ new owmer provided: ‘a, The current permittee notifies the Director by certified mail at least thirty (30) days in advance of the proposed transfer date; b. The notice includes the name and address of the new permittee and fa written agreement between the existing and new permittee containing a specific date for the transfer of permit responsibilty, surety and lieblity between them and; ¢. The Director does not notify the existing permittee ond the proposed new permittee of his intent to modify or reissue the permit, ‘The operator or person in responsible charge of the OWTS shall have a Rhode Island Class 1 (or higher) Wastewater Treatment Plant Operator Certification or an equivalent certification from another jurisdiction acceptable to the Director. If the operator is not employed directly by the permittee, a service agreement with the operator or other contact entity shall be entered into and continuously ‘maintained, and a copy of the active service agreement shall be submitted to the Director, “The permittee shall sample and test both the wastewater influent and effluent for the following parameters: Biochemical Oxygen Demand Total Suspended Solids ‘Total Nitrogen Nitrate Nitrite ‘Ammonia Fats, Oils & Grease Alkalinity PHPage 3 10. i. Temperature Total Daily Flow ‘The point of sampling of the influent shall be at the inlet of the 25,000 gation Equalization / Anoxic tank as shown on the plans. The point of effluent sampling shall be at the 15,000 gallon Dosing Tank in the treatment train on the plans. ‘Sampling and testing shall be conducted monthly. ‘The results shall be submitted in writing to the Director within thirty (0) days afte the end of the measurement period, Sampling and measurements shall be representative of the monitored activity. All sampling results shall be submitted to this department, ‘The permittee shall at all times properly operate and maintain all facilities and ipment installed, At least sixty (60 days prior to requesting approval to discharge any sewage into the proposed OWTS, the applicant shall cause to be prepared and submitted to the state for review and approval a copy of the ‘operation and maintenance plan. The plan shall specify procedures for operation, process control, record keeping, monitoring, weekly maintenance checks, other routine and periodic maintenance, safety and security. Also, the O & M plan shall include a contingency plan to address emergency actions and procedures in the event of equipment failure and power loss. ‘This approval is for the discharge of sanitary sewage only. No toxic, hazardous or industrial processing waste may be discharged into the system. Floor stripping wastewaters and other wastewaters generated at the facility that may adversely affect treatment shall be discharged to the floor stripping waste tank for appropriate disposal offsite. {All septage, sludges, screenings or other pollutants removed from the system shall bbe disposed in accordance with all applicable state and federal laws. ‘The permit may be modified, suspended, or revoked for cause. The Director reserves the right to revise the permit in order to ensure compliance with applicable state and federal regulations. ‘The Permittee shall allow the Director or his authe sd representative to access the OWTS system and work areas to inspect equipment, practices, records pertinent to this permit and stipulated compliance requirements, and to sample and monitor the wastewater for the purposes of determining compliance, In the event of treatment interruption, failure to treat to the discharge limits, bypass or sewage overflow, the permittee shall notify the Director verbally within twenty-four (24) hours. Such incident shall be reported in writing within five (5)Page 4 12, 13. days after it becomes known to the permittee, which report shall include the date and time(s) of the incident, the duration of the incident, and actions taken to correct the problem and prevent a reoccurrence, This project is proposed to be completed in 3 phases as submitted to DEM.
