on July 16, 2016 due to multiple gunshot wounds in ________________ City. Hereto attached is
a copy of the news article of the Visayan Daily Star dated ____________ relative to the reported
death of the aforementioned accused.

Mr. _____________ is the accused in Criminal Case No. ______________ for

Homicide which is pending before the Regional Trial Court (RTC), Branch
____________________. However, prior to ______________s death, neither he nor his family
came to our office in order to follow-up on the status of his case.
