    Dear Sir,

    I have started my anaesthesia appointment on 01.08.2016 under Dr. Hemantha


    Rajapakshe ( Consultant Anaesthetist NHSL ) and left on medical leave on
    16.08.2016. Now I have reported back to my appointment under same consultant
    on 01.05.2017 as instructed on the approval letter issued by PGIM.

    Since this is my last appointment for the 1st year, please be kind enough to
    advice me regarding the 2nd year appointment schedule.
