AFFIDAVIT OF ADMISSION OF PATERNITY

I, EGGY ANNIBAN, after having been duly sworn to in accordance with law hereby
depose and say:

1. That I am married, of legal age, Filipino citizen, and a resident of No. 27

Upper P. Burgos , Baguio City, Philippines;

2. That sometime in February 2008, I met Jenina Gunaden of legal age, Filipino
citizen and a resident of No. 52, New Lucban Baguio City, with whom I later
had a relationship;

3. That as a result of said relationship, she conceived and gave birth to a child
on December 21, 2009 and we named the child, SCARLET JEAN;

4. That I am the biological father of the child, Scarlet Jean;

5. That at the time of the birth of said child, I am not legally married to Ms.
Jenina Gunaden, her biological mother;

6. That I hereby admit under oath my paternity of the child, SCARLET JEAN;

7. That I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purposes it may serve.

IN WITNESS WHEREOF, have hereunto affixed my signature this 1st day of

February 2011 Baguio City, Philippines.

2011 by Eggy Anniban , personally known to me, who is the same person who personally
