Parent/ Guardian Waiver

I, Romeo R. Pea, of legal age and a resident of #94 M.L.Q. St. Purok 2, New Lower Bicutan,
Taguig City, hereby authorize the Polytechnic University of the Philippines Taguig Campus to
send my daughter, Mary Grace V. Pea, for the off-campus Field Study Observation at Bicutan
Parochial School and Taguig National High School High School, both located at Lower Bicutan,
Taguig City.

I hereby waive/renounce the right to any claim of compensation or course of action against the
PUP Taguig, Bicutan Parochial School and Taguig National High School in any case of accident
during the said activity.
