AFFIDAVIT OF LOSS

I, Jeffrey Labro, of legal age, and with residential address at Ubanon,

Catbalogan City, Samar , under oath, hereby depose and say, that:

1. That I am a graduate of St. Marys College of Catbalogan in 2005.

2. That I was issued quarterly report cards and a diploma and by the said
school.

3. That said report cards and diploma are missing and must have been lost
sometime in 2006 when we moved to Manila. Despite long and diligent efforts,
they could no longer be recovered.

4. I am executing this Affidavit to attest to the truth of the foregoing and to

support a request for replacement copy of the above-mentioned diploma and
report cards.
