I, JAYMARK I. AGBAYANI, of legal age, Filipino, single, and with

permanent residence at Blk. 1, La Paz, Tarlac, after being sworn to in
accordance with law do hereby depose and state, that:

1. My biological parents are JESSIE AGBAYANI(father) and

GERALDINE GABRIEL ISLA(mother);

2. I am not aware that my parents were never married;

3. My mother GERALDINE GABRIEL ISLA married DOMINGO ABAD

DELA CRUZ JR. on 4 December 1996 at La Paz, Tarlac;

4. I am executing this Affidavit to certify and attest to the truth of the

foregoing facts and for whatever legal purpose that this Affidavit
may serve.
