KATIE ADULT DAY CENTER POLICIES AND PROCEDURES

SUBJECT: Program and Individual Abuse Prevention Plans


Reference: Minnesota Statutes, section 245A.65, subdivision 2; and section
626.557, subdivision 14;

POLICY: An abuse prevention plan, covering all required elements, is established
and enforced. Individual abuse prevention plans, as required, are also
developed and implemented. The scope of the abuse prevention plan
for the program is limited to the population, physical plant, and
environment within the control of the license holder and the location
where licensed services are provided.

PROCEDURE:

1. The plan includes:


a. Population Assessment:
[Explain your assessment including the following factors: age, gender, mental
functioning, physical and emotional health or behavior of the clients; need for
specialized programs of care for clients; the need for training of staff to meet
identified individual needs; and the knowledge a license holder may have
regarding previous abuse that is relevant to minimizing risk of abuse for
clients].
b. Physical Plant Assessment:
[Explain your assessment of the physical plant where the licensed services are
provided, including the following factors: the condition and design of the
building as it relates to the safety of the clients; and the existence of areas in
the building which are difficult to supervise].
c. Environment Assessment:
[Explain your assessment of the environment for each facility and for each site,
including: the location of the program in a particular neighborhood or
community; the type of grounds and terrain surrounding the building; the type
of internal programming; and the programs staffing pattern].
d. For any areas of risk for abuse identified through these assessments, a plan will
be developed stating specific measures to be taken to minimize the risk of
abuse. [Explain here, the measures to be taken to minimize risk in any of the
identified areas above]. The plan shall comply with any rules governing the plan
promulgated by the licensing agency.
e. The plan will be implemented, and along with the governing body of the
Center, reviewed and revised, as appropriate, at least annually, using the

assessment factors and any substantiated maltreatment findings that occurred
since the last review.
f. Staff are oriented to the Center Abuse Prevention Plan.
g. Clients receiving services will be oriented to the Center Abuse Prevention Plan.
If applicable, the clients legal representative must be notified of the
orientation. Orientation will be provided for each new person within 24 hours
of admission, or for persons who would benefit from a later orientation, the
orientation may take place within 72 hours.
h. A copy of the plan is posted in a prominent location on the bulletin board, and
is also available upon request to mandated reporters, persons receiving
services and legal representatives.

INDIVIDUAL ABUSE PREVENTION PLAN


i. An individual abuse prevention plan for each vulnerable adult receiving services
will also be developed. The plan shall contain an individualized assessment of:
(1) the person's susceptibility to abuse by other individuals, including other
vulnerable adults; (2) the person's risk of abusing other vulnerable adults; and
(3) statements of the specific measures to be taken to minimize the risk of
abuse to that person and other vulnerable adults. For the purposes of this
paragraph, the term "abuse" includes self-abuse. The plan will be included in
the clients record. Staff will be oriented to the individual abuse prevention
plan for clients in their care.
j. If the center knows that the vulnerable adult has committed a violent crime or
an act of physical aggression toward others, the individual abuse prevention
plan will detail the measures to be taken to minimize the risk that the
vulnerable adult might reasonably be expected to pose to visitors to the facility
and persons outside the facility, if unsupervised. Under this section, a facility
knows of a vulnerable adult's history of criminal misconduct or physical
aggression if it receives such information from a law enforcement authority or
through a medical record prepared by another facility, another health care
provider, or the facility's ongoing assessments of the vulnerable adult.
