AFFIDAVIT

I, Chester L. Cabana, Filipino, 26 years of age, single and a resident of Purok 5, North
Poblacion, Medina Misamis Oriental, after having been sworn to an oath in accordance with
law, do hereby depose and say:

That I am an applicant for a teaching position within the Deped Misamis Oriental
Schools Division;

That as such, I was required to submit documents in support of my application for

employment with the Department of Education;

Than in compliance with the requirements, I have submitted copies of the following
documents, to wit:

a. Letter of Intent
b. CSC form 212 (revised 2005)
c. Certified photocopies of certificates of training attended
d. NBI clearance
e. Transcript of Records
f. General Weighted Average
g. Certified photocopy of registration/license
h. Certified photocopy of PRC professional ID card
i. Certified photocopy of certification of status of Employment
j. Omnibus certification of Authenticity and Veracity of Documents
That I am executing this affidavit to attest to the truth of the foregoing, the
submission of the above-mentioned documents in support of my application for a teaching
position with the Department of Education and for whatever legal purpose this may serve.
