Prognosis

A. Narrative

Scoring and Computation of Criteria

There are nine (9) criteria for the evaluation of the patients condition. Each
criterion has a total of 15 points for the 5 days of monitoring and caring for the patient.
Therefore 9 criterion multiplied by 15 points is equal to 135 points score. This score is to
be used to determine the health standing of the patient. And the percentage was taken
by dividing the patients score by the total score, and then multiplied by 100.Percentage
was taken to see the patients progression in the 5 days of monitoring.

Interpretation

The patient has scored 71 points out of 135 points, this implies that the patients
condition was progressively worsening and the possibility of recovery was leaning
towards the undesired. The total percentage of the prognosis is 52.59 %, which
presents a rather poor prognosis.

Score Range:

1-45 points (Poor health condition and is at high risk; chance of recovery: minimal)

46-90 points (Unstable health condition state and is at moderate risk; chance of
recovery minimal to moderate)

91-135 points (Stable and improving; high chance of recovery)


General Prognosis

Based on the data gathered and shown in the prognosis sheet, the proponents
interpretation to the result is that the patient, as to the time of our care, was in an
unstable health state and was sadly getting worse as the days went by. The condition
could have been controlled and stabilized, if the patient had been stable with the
treatment he was undergoing, however the patient was often restless and non-
compliant with the treatment and also the patients disease condition was compromising
factor in patients recovery this ultimately lead the patients termination.
