Neonatal Dengue Infection : Report of Dengue Fever in a 25 Days Old Infant

Abstract
Background Dengue is an endemic tropical disease and a major contributor of health problem in
Indonesia. The incidence has been increasing consistently in this last decade. DHF/DSS is very
rare in the neonate and reported cases are few. We report a 25 days old baby who developed
primary DHF manifestation.
Objective Report a case of a 25 days old baby with primary DHF.
Case Report A 25 days old term neonate with 4560 gr weight was admitted with 2 days of fever.
The baby was active, allert and tolerating breastfeeding. There was no history of rash, convulsion
or bleeding from any site. On examination the heart rate was 140/min, respiratory rate 48/min,
temperature 380C with normal perfusion. Investigation revealed hemoglobin 14,6 g/dl, total
leukocyte count 7.2x103/uL, platelet count range 4.000mm3-83.000mm3, haematocrit range 23%-
37% and positive NS-1 antigen. She was transferred to special care nursery for intensive
monitoring. She was treated with fluid maintenance 150cc/Kg/day (IV and oral), Cefotaxim for 7
days for possibility of septicemia and platelet transfusion for prevent of severe bleeding. She was
discharged well after 9 days hospitalized
Conclusion Dengue infection is rare yet a considerably important infection in neonatal period.
Symptomatic and supportive treatment under close observation is the mainstay of treatment, thus
it is important of raising awareness of possible neonatal dengue for optional treatment especially
in endemic areas like Indonesia.
