(3) It is again highlight that QAP Plan has been not covered the all details as per NHAI Quality Assurance manual 2006. In most of the point has been indicated to be rectified as mentioned below:- ‘© The organization chart of Concessionaire, EPC Contractor & PMC should be given separately. + The name of difference executive agency/vendors engaged for different works should be given separately. ‘© The format of RFl and flow chart of RFI should also be mentioned. «In most of the Quality Test Report formats relevant IRC Code reference has not been mentioned. Ps ‘Flow Chart for DLC/PQC should also be included © Flow chart for Highway maintenance (Pot holes, Patch repair, Median Cleaning, drain Cleaning , Culvert Cleaning etc) «A Format for all tests conducting for different items of work including quantity of work done during the month may also be included. The check list should be as per NHAI Quality Assurance plan 2006 to be attached. © Check list for maintenance activities are to be included ‘+ Inspection Plan of Highway & Structure works each and every item to be included in QAP. ‘= QAP Submitted by you without numbering and without authorized signatory please revised ‘QAP to be submit with rectification. In Quality assurance plan submitted by you no where the name of concessionaire, PMC, EPC Contractor has been mentioned. Apparently it appears that L&T has prepared the QAP which keeping in mind the CA Provisions and NHAI Quality Assurance manual 2006 You may further modify/Improve the QAP further at your end if any things missing to make the preparation better Please resubmit within 3 days urgently. ‘Thanking You, bhr—~ KK.
