To whom it may concern,

I, Naomi Isabelle P. Zulueta, hereby authorize my friend, Rodolfo I.

Sales Jr. to claim my authenticated NSO birth certificate on my behalf
because I cannot go there personally because I have classes to attend in
school. I need it on today for my completion of visa requirements, so I do
not have any more time to claim it personally.
