Sir or Madam,

Hello! I am reaching out to you in regards to my patient, male, 32 years old. Patient is
displaying night sweats, weight loss, itchy skin, fatigue, back pain, raised levels of the protein
CDR2, gait unsteadiness, dizziness, swollen lymphnodes, lumps under skin, tremor, and
dysarthria. Patient has no history of chronic illness, other than a short bout of Ebstien-Barr in the
7th grade. Patient missed approximately 4 weeks of school during that time. We are unsure of the
diagnosis, why his symptoms are occurring, treatment, what organs are impacted, and what will
happen if the patient is not treated.

We need your expertise. You must present to our diagnostics team before treatment can
begin. The diagnostic team has high expectations and needs very clear evidence before they will
allow us to treat the patient. You will need evidence for why a symptom is occurring, what the
diagnosis is, what the treatment is, why the treatment will work, long-term impact of treatment
and what the outlook is for the patient. Simply stating the diagnosis will not get your proposal
approved, you must explain exactly what is happening inside and outside of the patients body.
Be clear, use research, evidence, diagrams, models, anything that will help convince the board of
your diagnosis.
