1. Reference: Memo received from OIC, RID, PRO 10 with RN PRO10-RID3
1708-02 dated August 2, 2017 regarding the submission of illegal
drug personalities categorized as Cannot be located.

2. Be informed that the submission of illegal drug personalities categorized as

Cannot be located with corresponding certifications from BADAC and MADAC has
been lapsed.

3. It was observed that you only submitted the BADAC and without the MADAC
resolution. Further, be reminded that the latter is very important.

4. Therefore, you are directed to submit an explanation in affidavit form,

subscribed and sworn to before a notary public, stating thereat, why no disciplinary
action that would be held against you.

5. Submit to this office NLT 1:00 PM, Monday, August 7, 2017 in hard copy
form.

Note: MADAC resolution is our concern, please submit as provided.
