Philosophy and Policy
SLC Sports Complex Ice Area strives to provide a state-of-the-art, ADA accessible
recreational facility, where figure skaters, hockey players, and recreational skaters alike
can enjoy a safe, family-friendly, and professionally managed environment. To provide
the best and most reasonable Standard of Care possible, we will use only qualified and
trained employees. Moreover, we endorse the establishment of a program to manage
risks associated with the Ice Arena to protect the safety of everyone within our
organization.

Although there is an element of risk inherent in participating in any ice-related


activity, we are dedicated to maintaining and promoting a safe and healthy environment
for staff and users. We expect users to acknowledge and appreciate this face, and
we operate under the following policy:

Assumption of Individual Risk


All participants using the SLC Sports Complex Ice Arena are required to check in/
slide membership card at the front desk and sign a form that states the assumption of
risk prior to participating in programs offered. SLC Sports Complex will not be held
liable for any damages to person or property (including injury and death) resulting from
participation in any of its programs, including but not limited to:

Private and group figure skating lessons and freestyle sessions.


Private and group hockey lessons and games.
Public skating.

It is the responsibility of participants to understand and assume liability for the


risks and dangers inherent to ice-related sports and recreational activities. These risks
and dangers include, but are not limited to:

Minor injuries such as scratches, bruises, and sprains.


Major injuries such as eye injury, or loss of sight, joint or back sprains, strains,
breaks, concussions, sever cuts, head trauma, dismemberment, cardiac arrest,
partial or total paralysis and death.
SLCSC General Site Policies
We have implemented several site policies in order to best provide patrons with a
safe, family-friendly, and professionally managed environment. We believe these policies
facilitate safe environments for all users and benefit their recreational experience.

In compliance with the Utah Clean Air Act smoking and smokeless
tobacco is prohibited in or around the building.
All patrons and visitors must check in/slide membership card at the
front desk.
Food and beverages are not allowed in the fitness areas, with the
exception of capped, spill-proof, non-glass water bottles.
Possession of illegal drugs, beer, and alcoholic beverages is
prohibited on the premises.
Foul and abusive language is unacceptable.
Abuse and/or misuse of County property and equipment is
unacceptable. Violators will be held financially responsible for
damages and losses. Any area in which the above rules are violated
will be subject to closure. Violators will be asked to leave the
premises and action may result in entry privileges being suspended
or revoked.
No bare feet allowed outside of the locker rooms.
Lockers are for day use only. Lockers which are not cleared at the
end of the day will have the lock cut off and all items will be
revoked and bagged for pick-up. There will be a two (2) week
holding period at which time all items found will be sent to an
establishment which collects and resells used merchandise.
Patrons are fully responsible for their personal items that are lost,
stolen, or damaged at the Center. Books, bags, coats, and personal
items should be kept in a locker and are not allowed in activity
areas. Patrons are responsible for bringing their own lock when
using the lockers.
Patrons may not display actions that the Center Staff deem
unsportsmanlike or rude.
Misuse, moving, or alerting any portion of the Center property is
prohibited.
Persons with infectious diseases are prohibited from using the
facility. Open wounds must be covered.
All children under the age of ten (10) must be accompanied by an
adult (age 18 or older) in all areas of the facility at all times.
Please use only the parking areas immediately adjacent to our
building.
Patrons and visitors shall abide by the rules and regulations of the
Salt Lake City Sports Complex. People found in violation of these
rules may have their privileges suspended or revoked.

Step 3:
Goals and Objectives
1. Develop a comprehensive risk-management plan that will encompasses all SLC
Sports Complex Ice Arena departmental operations.

a. By the middle of 2016, obtain policy direction from Katelyn Chambers


pertaining to the development of the risk management plan.
b. By March 15 2016, appoint Risk Manager Coordinator.
c. Inform entire staff by June 1 2016, of the development of a comprehensive
risk-management plan.

2. Create a detailed plan that will reduce rick for employees, users, the organization,
and the facility.

a. By March 5, 2016 all potential facility and property hazards will be

b. By March 10, 2016 all SLC Ice Arena programs will be identified and added
to the risk management plan.

l. By April 20, 2016, we will contact and have compete legal insurance
counselor available.
m. By April 25, 2016, we will establish procedures and a timetable to review
our risk management plan and ensure that our facility if functioning at the
most reasonable standard of care possible.

Step 4:
Site and Facility Development
SLC Sports Complex, as a member of Salt Lake County Parks & Recreation, is in
compliance with title II of ADA. Dedicated to providing the highest standard of care, all
services, programs, and/or activities at both the site and the Ice Arena are accessible to
individuals with disabilities.

Parking Lot
Parking is located immediately adjacent to SLC Sports Complex, on the West side.
There are two lots for those with permits, accessible parking is available 10 to 15ft from
the curb ramps.
Entrance
Patrons access SLC Sports Complex through four (4) doors at the west entrance,
two of which are ADA Accessible.

Lobby
Patrons enter the site and walk through the lobby. The benches provided are
constructed of wood instead of metal to reduce the risk of injury from sharp metal
edges. The front desk is equipped with two different gates in order to ensure all patrons
have access to the facilities.
Rentals

Ice Skate rentals are adjacent to the West Ice Arena doors.

West Ice Arena


Most patrons use the West Ice Arena and enter through these doors.
East Ice Arena

The East Ice Arena is rented out and used for programs other than
public skating.
Ice thickness: Ice must be kept between 1 and 1.5 inches thick, enough to
support skaters weight and at the same time keep electrical costs to a
minimum. Any thinner and the ice is in danger of cracking, which can result in
injury to skaters and employees and financial loss to repair surface. Ice must
be checked once per week by a trained staff member. Using a cordless drill
and tape measure, the employee will check the ice in several specified
locations and record the finding on the appropriate Ice Thickness Chart. Any
need for thinning or thickening can then be addresses by rink maintenance
staff before skaters take to the ice.
Ice Surface: Providing a smooth skating surface free of dents, holes, and
bumps that skaters may trip over requires that the ice be resurfaces every 1
hour session. During resurfacing, to prevent staff or user injury, no one will be
allowed on the ice with the Zamboni except the Zamboni driver. To prevent
injury, rink entry must remain closed at all times while the Zamboni is on the
ice and am employee should remain at the gate as an extra precaution to
prevent access to the rink. Guest should only be open when the Zamboni has
left the ice, staff has finished inspecting the rink surface and patching holes,
and the Zamboni gate has been closed.
Rink Entry: Inadequate entry/exit points could result in injury in the event of
an emergency. Rink entry gates should remain open at all times that skaters
are on the ice in case the ice has to be evacuated.
Air Quality: If building ventilation is inadequate, Zamboni exhaust and other
chemicals could pose health risks for rink users. Proper ventilation if of upmost
importance in order to protect rink users and staff and ensure appropriate
health standards are being met.
Rink Boards and Netting: To prevent injury from stray hockey pucks, the rink
is surrounded by Plexiglas and netting.

Ice Arena Extension Areas
Parking Lot: To prevent injury resulting from tripping, crack in asphalt and
concrete will be repaired within two business day after discovery. Snow
removal in these areas will occur before 9:00am the morning following a
storm. All outdoor walkways will be salted in the event of freezing weather.
Front Desk: The admissions desk will attended by two employees at all times
to ensure guests will have someone to speak with about information if/when a
call comes in.
Skate Rentals: The rental office is to remain locked after business hours to
prevent theft and injury from equipment.

Lobby:
Slip: Flooring will be made of rubberized material to minimize the risk of
patrons slipping on wet floors
Security: Items left unattended have the potential for being stolen and
therefore propose a security risk. Those items will be removed from the Lobby
immediately by the staff and placed in the lost and found.

Facility Equipment
Poorly Maintained Equipment: Malfunctioning equipment including the
score board and PA system poses financial risks to SLCSC Ice Arena. This
equipment will be operated by trained employees only, and will be checked
monthly to ensure smoother operation and avoid long-term issues and costly
replacements.

First Aid and Emergency Equipment


First Aid Kits: To ensure quick response and minimize severity of injury in case
of emergency, a first aid kit will be available in both ice arenas, at the rental
counter, and front desk areas. Employees will be aware of the location and will
be trained on how to properly use the kit. In the event of an emergency where
the use of the kit is required, it will need to be restocked appropriately at a
later time.
Emergency Equipment: Emergency equipment including fire alarms,
sprinklers, PA systems, and emergency exit signs may malfunction occasionally.
To ensure that all emergency equipment is in in working properly, it will be
serviced in according to safety standards.

Zamboni
Zamboni Access: To prevent user and unqualified employee access and
potential injury, the Zamboni will be kept in the locked Zamboni room; only
employees trained to operate the Zamboni should have key to the room.
There will be posting of clear, visible warning signs to reinforce the rules and
dangers of using the Zamboni.
Zamboni Blade: The Zamboni Blade will be changed with caution once a week
in order to ensure smooth, quality ice and minimize the risk of user injury.

Salt Lake City Sports Complex Ice Rink Emergency Procedures

MEDICAL EMERGENCY

In the event of a medical emergency, follow this action plan:


1. Have first responder gather the following information: i. Is the person breathing? ii. Is the
person conscious? iii. Does the person have circulation? iv. How old is the person? v. What sex is
the person? vi. What is the suspected problem?

2. Dial 911 i. Answer all the dispatchers questions ii. Dont hang up iii. Help them to guide the
ambulance to the facility

3. Render any First-Aid that is necessary and reasonable

4. Have an employee standing by to help guide paramedics to the patient.

5. Provide crowd control

FIRE ALARM

In the event of fire alarm follow this procedure:

1. Begin escorting patrons out of the facility i. Do not scream fire, remain calm ii. Make
announcement over the PA for people to calmly leave

2. Look for smoke of flames in your area only

3. Have supervisor check fire alarm panel for location of alarm

4. If no smoke or flame is visible, check area of alarm i. Only do so if you can do it safely

5. Dial 911 if you are unsure of whether there is a fire or not

6. If there is no fire, reset alarm and call Mountain Alarm at 363-9696 and give them your
password:

7. Allow patrons back inside if the building is determined to be safe.

POWER OUTAGE

In the event the electricity goes out, use this procedure:

1. Look outside to see if the street lights or neighboring buildings are without power i. If they
are, it is a local outage and we cannot do anything. Instruct patrons that we are trying to get
the power back on and to be patient

2. If there is public skating session underway, the ice must be cleared in a safe and orderly
manner.

3. If the power outage is isolated to our building, call Wayne Johnson (Parks Maintenance) for
instructions, or call Clark Littleton, or another Merit employee.
4. We have a back-up generator that will provide enough light to allow people to stay in the
building.

EXPLOSION/CHEMICAL LEAK/EARTHQUAKE NATURAL DISASTER

If an explosion or chemical leak occurs do the following:

1. Dial 911 immediately

2. Remain Calm

3. Evacuate building. Use PA system to direct people to exits

4. If there is an ammonia leak, open all outside doors.

5. Render any first aid that is possible

LOST CHILD/KIDNAPPING

If a child is reported missing follow these guidelines:

1. Gather Identifying information about the lost child i. Age ii. Sex iii. Height iv. Weight v. Clothes
vi. Hair Color

2. Where was the child last seen and with whom?

3. Radio description to employees in facility

4. Notify Police via 911

5. Watch all exits and begin thorough search of building

6. DO NOT PUBLICALLY ANNOUNCE THERE IS A LOST CHILD

EVACUATION PLAN

In the event we must evacuate the building, do the following:

1. Remain calm, and do not panic. If we panic, then the public will

Complex, its employees related to this activity is in the Superior Court of Salt Lake, Utah.
In the case of a minor Participant, the Undersigned parent or legal guardian acknowledges that
he/she is not only signing this agreement on his/her behalf, but that he/she is also signing on
behalf of the minor and that the minor shall be bound by all the terms of the Agreement.
Additionally, by signing this Agreement as the parent or legal guardian of a minor Participant,
the parent of legal guardian understands that he/she is also waiving certain rights on behalf of
the minor that the minor otherwise may have. The Participant would not be permitted to
participate in the Activity.

ASSUMPTION OF RISK, LIABILITY RELEASE, INDEMNIFICATION AND REFUND POLICY

1. Release and indemnification: I hereby recognize and acknowledge that my or my child(ren)s participation in recreational activities
may involve bodily and /or emotional injury to myself and/ or my child. In consideration of me or my child(ren) being able to
participate in such events. I, for myself my child(ren), my heirs, my executors and administrators, hereby voluntarily and knowingly
indemnify and hold harmless, defend, release, waive and discharge Salt Lake County and its officers, employees and volunteers
from an and all suits, claims or liability, including negligence, based on any injury except that caused solely by the willful misconduct
of Salt Lake County, that my result from my or my child(ren)s participation in Salt Lake County Parks and Recreation activities. In
addition I agree that I or my insurance company will pay fro medical, hospitalization, or any other expenses resulting from my or my
child(ren)s participation.
2. Refund Policy: As per Salt Lake County Policy and Procedures, prior to the first day of the program and/ or prior to usage of the
facility, the Parks & Recreation Division may withhold 25 % of the refund for administrative cost. All refund must be requested in
person and accompanied with a written refund request. No refund will be given after the first day of the program, and or usage of the
facility, except as provided in Salt Lake County Policy and Procedure #1205.
3. Collections: I agree to pay Salt Lake County all cost incurred, together with reasonable attorneys fee in the event that my account
is referred the Salt Lake County Attorneys Office for collection. I understand that any account delinquent 30 days or more will be
turned over to the Salt Lake County Attorney for collection.
4. Emergency Treatment: I hereby authorize Salt Lake County Parks & Recreation program staff to act on my behalf in accordance
with their best judgment in case of an emergency involving me or my child(ren), and agree to assume full responsibility for all
expenses, medical or otherwise, that may arise there from. I understand that I or my insurance company will pay from such
emergency treatment.

CONDITIONS OF THE LICENSE

1. The instruction of ice skating lessons at any of the above listed ice rinks may only be done pursuant to the terms of this license
agreement.
2. Rates for Licensee to access the facility to teach ice skating lessons may be paid either on a monthly basis or on a per use
basis as follows:
$120.00 PER MONTH for each month licensee uses the Facilities or
$5.00 per freestyle/public hourly session.
3. Payment Terms
a. For licensees intending to pay monthly payments are due by the 5th day of the month or upon the first day of coaching
if that day is after the 5th of the month. Accounts past due 60 days will be turned over for collections. Licensee agrees
to pay Salt Lake County reasonable attorney fees in
the event Licensees account is referred for collection.
b. Hourly payments are due the date of use, prior to use of the Facilities.
c. Payment in full must be received at the front desk of the primary facility Licensee intends to use.
4. Licensee Obligations
a. During the term of this License, Licensee must be a member of Professional Skaters Association (PSA) and/or
registered with the United States Figure Skating Association (USFSA) or the Ice Skating Institute (ISI), and participate
in such skills training as may be required by the specified association or organization in order to retain appropriate
certification.
b. Licensee may use the Facilities only during designated hours as determined at the Countys sole discretion, and
subject to change without notice solely in Countys discretion.
c. Licensee shall comply with the Countys Ice Rink Rules as posted at the Facilities, which are hereby incorporated
by reference. Licensee shall also comply with all applicable laws, orders, ordinances, rules, regulations, relevant
class/program standards, and requirements of federal, state, or city government. County shall always have the right
to remove any person from the Facilities.
d. Licensee is responsible to notify students and parents of the rules discussed in Paragraph 4(c) of this License.
Licensee is responsible to notify their students of appropriate ice fees including admission and skate rental fees.
e. Licensee agrees not to sell, distribute or solicit the sale or distribution of any material, equipment, or other goods
whatsoever, in or about the Facilities without prior written consent of County.
f. Licensee understands that County will not sponsor or endorse any activity, program, or performance outside the
Facilities without prior written consent of County. Licensee agrees not to conduct promotional activities or advertise any
special event at County facilities without prior written consent of County.
g. Licensee is not authorized to use any of Countys business equipment, materials, etc.
5. Equipment and Facilities
a. The Licensee shall provide and maintain all required personal equipment and class/program equipment and supplies
at his/her own expense. It is specifically understood and agreed that County is not required to provide equipment,
supplies or materials to the Licensee.
b. County will provide access to its hockey nets, sound system, and jumping harnesses, subject to availability.
c. County will provide adequate locker room facilities to Licensee. Licensee agrees to abide by the rules and
regulations for locker room use set forth by County, as well as the policies and procedures for room key check-
out as set forth by County.
6. Liability
a. Licensee agrees to indemnify, hold harmless and defend County, its officers, agents and employees from and against
any and all losses, damages, injuries, liabilities, and claims, including claims for personal injury, death, or damage to
personal property or profits and liens of workmen and material men (suppliers), however allegedly caused, resulting
directly or indirectly from, or arising out of, negligent acts or omissions by Licensee, its agents, representatives,
members, guests, trainees, students, and competitors in the performance of this License.
b. County is a body corporate and politic of the State of Utah, subject to the Governmental Immunity Act of Utah (the
Act), Utah Code Ann. 63G- 7-101 to -904 (2011). The parties agree that County shall only be liable within the
parameters of the Act. Nothing contained in this License shall be construed in any way, to modify the limits of liability
set forth in that Act or the basis for liability as established in the Act.
c. Licensee assumes liability for damage to County property caused by Licensee, his/her agents, representatives,
members, guests, trainees, students, and/or competitors during each lesson / session.
d. Licensee assumes all risk of loss in the event this License is terminated. In no event shall County be liable for any costs
or attorney fees expended by the Licensee in enforcing his/her rights under this License. The Licensee agrees that
County shall not be liable for indirect, incidental, or consequential damages, regardless of the form of action, nor shall
it be liable for exemplary damages or lost revenue.
e. County shall not be responsible for losses by Licensee, its agents, employees, members, guests, or invitees occasioned
by theft or disappearance of equipment or other personal property.
7. Term and Termination
a. This License shall be effective upon execution by both parties
and shall terminate (check one) After one calendar year

On (date), but no later than one year from the date of execution.

b. To continue to instruct lessons at a County Facility, Licensee shall be required to sign a new license agreement upon
the termination of this Agreement.
c. Termination for Convenience. Either party may terminate this License, in whole or in part, at any time whenever the
terminating party determines, in its sole discretion that it is in its interest to do so. The terminating party shall provide
written notice at least 30 (thirty) days prior to the date of termination. The parties agree that termination will not entitle
the non-terminating party to any rights or remedies provided by law for breach of contract or any other claim or cause of
action.
d. Termination for Default. County may terminate this License upon ten day written notice, if, in the sole discretion of County,
the Licensee fails to comply with Paragraphs 3, 4(b), (d), (e), (f), or (g) of this License.
e. Immediate Termination. County may terminate this License immediately, if, in the sole discretion of County, the Licensee
fails to comply with Paragraphs 4(a) or 4(c) of this License.
f. Upon termination, Licensee agrees to pay in full for use of the Facilities up to the date of termination, and quit and
surrender use of the Facilities for coaching purposes. Upon termination under Paragraph 7(c), (d), or (e) of this License,
County will refund any unused portion of the fee on a pro-rata basis. Upon termination of this License, Licensee shall
leave the Facilities in a clean condition.
8. Miscellaneous Provisions
a. This License many not be assigned or transferred without prior written consent of County.
b. No agent, employee or servant of Licensee or County is or shall be deemed to be an employee, agent or servant of the
other party. None of the benefits provided by each party to its employees including, but not limited to, workers
compensation insurance, health insurance and unemployment insurance, are available to the employees, agents, or
servants of the other party. Licensee and County shall each be solely and entirely responsible for its acts and for the
acts of its agents, employees, and servants during the performance of this License. Licensee and County shall each
make all commercially reasonable efforts to inform all persons with whom they are involved in connection with this
License to be aware that Licensee is an independent contractor.
c. All notices to be given under this License shall be made in writing and shall be deemed given upon personal delivery,
upon the next business day immediately following the day sent if sent by overnight express carrier, or upon the third
business day following the day sent if sent postage prepaid by certified or registered mail, return receipt requested, to
the parties at the following addresses (or to such other address or addresses as shall be specified in any notice given):

9. Standard Form. Any alteration of the standard form language without approval of the attorney shall render this agreement
void and without effect. Any changes to this agreement must be pre-approved as to form by the District Attorneys Office.

Step 7:
Establishing Safety Rules, Regulations, and
Procedures
Ice Rink Rules

No use of electronic devices during skating sessions, as they can


cause distractions and lead to injury.
No fast or carless skating.
No more than three (3) persons allowed to hold hands while
skating.
Jumps and spins allowed only at center rink or designated areas
during public skate.
No one allowed on ice during resurfacing.
No food or beverages allowed on Ice.
No horseplay or potentially unsafe games allowed on ice.
i) Tag, or whip.
Carrying children on the ice is prohibited.
Skaters must skate with the flow of traffic.
Sticks and pucks are allowed only during designated times.
Helmets are required when sticks and/or pucks are present on the
ice.
No shoes or boots allowed on Ice.
Skates re not allowed on the bleachers.
All skating instructors must show qualifications. (Ask how they hire
their skating teachers. I think they hire them through another
agency.)
User groups are responsible for cleanup, damages, and conduct of
participants and spectators on and off the ice during private facility
rentals
Those participating in facility activities assume all inherent risk and
responsibilities whether spectator or participant.
SLC Sports Complex is not responsible for unattended, lost or
stolen articles.
Inappropriate use of profanity, tobacco, alcohol, or sunflower seeds
may be asked to leave and be suspended from any activity at the
facility for a person of tie at the discretion of facility management.
Management is not responsible for lost or stolen articles.
Posted in the staff lounge is a bulletin board displaying information to employees.
Step 8:
Safety Inspection & Investigation
SLC Sports Complex Ice Arena recognizes that its facility and equipment require
inspections and maintenance to protect the safety of patrons and staff. All staff
information, qualifications, accident/incident reports, and maintenance documentation
will be filed in an administrative office for safe keeping and will be kept on record for
seven (7) years. The following refers to areas requiring inspection and how often they
should be completed:

Step 9:
Accident Reporting and Analysis

Within 24 hours of an accident/incident, a SLC Sports Complex staff member will


complete a written Accident Reporting. Reports will be completed objectively unbiased,
with enough detail for analysis and appraisal. Do not ask for opinions on how the
accident/incident could have been prevented. Within a week, a manager will contact the
injured party and complete an Accident Report Follow Up form. The forms will be
stapled together and filed according to location of accident. The completed forms will
be evaluated every quarter to find patterns, and adjustments will be made to programs.

If an accident occurs at the SLC Sports Complex Ice Arena:

Call 911 if there is a serious bodily injury or significant property damage.


Notify a manager as soon as possible.
The manager should notify the rink administrator
o Immediately if there is serious injury or significant property damage.
o Within 24 hours if no serious injury or significant property damage.
Step 10:
Emergency Procedures

I. MEDICAL EMERGENCY
In the event of a medical emergency, use this action plan:

1. Have first responder gather the following information:


a. Is the person breathing?
b. Is the person conscious?
c. Does the person have circulation?
d. How old is the person?
e. What is the suspected problem?

2. Dial 911.
a. Answer all the dispatchers questions.
b. Dont hang up.
c. Help them to guild the ambulance to the facility.

3. Render any First-Aid that is necessary and reasonable.

4. Have an employee standing by to help guide paramedics to the patient.

5. Provide crowd control.

II. FIRE ALARM


In the event of a fire alarm follow this procedure:
1. Begin escorting patrons out of the facility.
a. Do not scream fire, remain calm.
b. Make announcement over the PA for people to calmly leave.

2. Look for smoke of flames in your area only.

3. Have supervisor check fire alarm panel for location alarm.

4. If no smoke or flame is visible, check area of alarm.


a. Only do so if you can do it safely.

5. Dial 911 if you are unsure of whether there is a fire or not.

6. If there is no fire, reset alarm and call Mountain Alarm at 363-9696 and give
them your password:_______

7. Allow patrons back inside if the building is determined to be safe.

III. POWER OUTAGE


In the event the electricity goes out, use this procedure:

1. Look outside to see if the street lights or neighboring buildings are without
power.
a. If they are, it is a local outage and we cannot do anything. Instruct patrons
that we are trying to get the power back on and to be patient.

2. If there is public skating session underway, the ice must be cleared in a safe
and orderly manner.

3. If the power outage is isolated to our building, call Wayne Johnson (Parks
Maintenance) for instructions, or call Clark Littleton, or another Merit
employee.
4. We have a back-up generator that will provide enough light to allow people to
stay in the building.

IV. EXPLOSION/CHEMICAL LEAK/EARTHQUAKE


NAUTURAL DISASTER
If an explosion or chemical leak occurs, do the following:

1. Dial 911 immediately.

2. Remain calm.

3. Evacuate the building. Use PA system to direct people to exits.

4. If there is an ammonia leak, open all outside doors.

5. Render any first aid that is possible.

V. LOST CHILD/KIDNAPPING
If a child is reported missing follow these guidelines:

1. Gather Identifying information about the lost child.


a. Age
b. Sex
c. Height
d. Weight
e. Clothes
f. Hair color

2. Where was the child seen last and with whom?

3. Radio description to employees in facility.

4. Notify Police via 911.

5. Watch all exits and begin through search of building.

6. DO NOT PUBLICLLY ANOUNCE THERE IS A LOST CHILD

VI. EVACTUATION PLAN


If the event we must evacuate the building, do the following:

1. Remain calm, and no not panic. If we panic, then the public will.

2. If possible, make a calm announcement over PA system or with cordless PA


system.

3. Follow Fire Alarm Evacuation Plan.


Step 11.
Releases, Waivers, and Agreements to Participate
The waivers are to protect our recreation center from any possible legal issues
that may arise. Participants will be required to read, sign, and date the waivers before
they use our ice rink for any activity. Signing this waiver means the participant is
assuming any and all risks that may arise while using the ice rink. After the participant
signs the waiver, they will be kept in the administration offices.

Undersigned, acknowledge that I have carefully read this assumption of tick and release of liability and I
understand its contents. I understand that my signature below expressly waives any rights I have to sue
SLC Sports Complex for injuries and damages related to this ice skating activity.

I hereby agree that the exclusive jurisdiction for any claims I may bring against SLC Sports Complex, its
employees related to this activity is in the Superior Court of Salt Lake, Utah.

In the case of a minor Participant, the Undersigned parent or legal guardian acknowledges that he/she
is not only signing this agreement on his/her behalf, but that he/she is also signing on behalf of the
minor and that the minor shall be bound by all the terms of the Agreement. Additionally, by signing this
Agreement as the parent or legal guardian of a minor Participant, the parent of legal guardian
understands that he/she is also waiving certain rights on behalf of the minor that the minor otherwise
may have. The Participant would not be permitted to participate in the Activity.

ASSUMPTION OF RISK, LIABILITY RELEASE, INDEMNIFICATION AND REFUND POLICY

1. Release and indemnification: I hereby recognize and acknowledge that my or my child(ren)s participation in recreational activities
may involve bodily and /or emotional injury to myself and/ or my child. In consideration of me or my child(ren) being able to
participate in such events. I, for myself my child(ren), my heirs, my executors and administrators, hereby voluntarily and knowingly
indemnify and hold harmless, defend, release, waive and discharge Salt Lake County and its officers, employees and volunteers
from an and all suits, claims or liability, including negligence, based on any injury except that caused solely by the willful misconduct
of Salt Lake County, that my result from my or my child(ren)s participation in Salt Lake County Parks and Recreation activities. In
addition I agree that I or my insurance company will pay fro medical, hospitalization, or any other expenses resulting from my or my
child(ren)s participation.
2. Refund Policy: As per Salt Lake County Policy and Procedures, prior to the first day of the program and/ or prior to usage of the
facility, the Parks & Recreation Division may withhold 25 % of the refund for administrative cost. All refund must be requested in
person and accompanied with a written refund request. No refund will be given after the first day of the program, and or usage of the
facility, except as provided in Salt Lake County Policy and Procedure #1205.
3. Collections: I agree to pay Salt Lake County all cost incurred, together with reasonable attorneys fee in the event that my account
is referred the Salt Lake County Attorneys Office for collection. I understand that any account delinquent 30 days or more will be
turned over to the Salt Lake County Attorney for collection.
4. Emergency Treatment: I hereby authorize Salt Lake County Parks & Recreation program staff to act on my behalf in accordance
with their best judgment in case of an emergency involving me or my child(ren), and agree to assume full responsibility for all
expenses, medical or otherwise, that may arise there from. I understand that I or my insurance company will pay from such
emergency treatment.

Step 12.
Methods of Insuring Against Risk
At the Salt Lake City Sports Complex Ice Arena we will work and stay in contact
with our insurance provider to ensure that we are properly insured. We will consult with
our insurance provider a minimum of two times every fiscal year to review our insurance
policy. We will consult with our insurance provider before bringing any new programs
involving the ice rink to make sure we are properly covered in our insurance policy.
Step 13.
In-Service Training
Goal: Through in-service training we plan to ensure that all our Salt Lake City
Sports Complex Ice Arena instructors, facility maintenance, and coordinators are
knowledgeable about the rules, regulations, and policies. The success of this goal will
result in reducing the risk associated with any activity from using our ice area.

Objective: Within three weeks of being hired, our instructors, and coordinators
will know the following procedures in order to provide the best quality of care for our
participants and their family members.

o Newly hired instructors and coordinators must be knowledgeable about handling


situations in emergencies.
o Newly hired instructors and coordinators must be knowledgeable about the
organizational structure of the ice area.
o All instructors and coordinators, newly hired or seniority, must be aware of all
emergency exits connecting to the ice area.
o All instructors and coordinators must be knowledgeable of co-instructors names
and positions to be able to guide others of who to speak to in their situations.
o All instructors and coordinators must be knowledgeable of hours of operation for
all activities involving the ice area.
o All instructors and coordinators must be knowledgeable of how to handle
participants and their families members complaints in a respectful manner.
o All instructors and coordinators must be knowledgeable on their duties and
responsibilities per position they were hired for.

To ensure that our instructors and coordinators have access to our rules, policies,
regulations, and their duties in their positions, we will have an employee manual in
the administration office as well as the ice area office.
Step 14.
Public Relations
At the Salt Lake City Sport Complex Ice Arena we are aware of the risks that may
be associated with the use of our ice rink. We do as much as we can to ensure that these
risks do not became a reality to our participants. In the chance that we do we will act in
a way of compassion, concern, and consideration for our participants.

The Salt Lake City Sport Complex Ice Arena public relation policies are the
following:

All our staff members involved with the Ice Area will be knowledgeable about all
policies and rules.
All our staff members will be knowledgeable on emergency procedures.
All our staff members will treat other staff members with respect.
All our staff will be qualified and trained instructors.
All staff with leader ship responsibilities will follow and enforce the rules of the
Ice Arena.
All our staff members will treat participants and participants family members
with respect and give them the best quality of customer service.
We require that all participants read, sign, and date waivers and agreements to
participate before using our Ice Arena.

All of the Salt Lake City Sport Complex Ice Arena public relations questions dealing
with incidents that occur during the activities will be turned over to the facility public
relations manager. The current public relations manager is Callie Birdsall. If you have
any questions regarding incidents or injuries related to the Ice Area please call Callie
Birdsall at 385-226-7177.
Step 15.
Outside Specialist/Legal Insurance
At Salt Lake City Sports Complex Ice Arena we will always retain and consult with
an outside legal counsel with matters that pertain to the risk associated within our
facility.
Step 16.
Periodic Review
At the Salt Lake City Sports Complex Ice Arena we pledge to review our risk
management plan a minimum of twice a year, every six months starting on the date of
March 1st. Our plan to review the risk management plan will include risk management
consultant, facility director, figure skating coordinator, hockey coordinator, and the
youth program manager. To ensure our staff know the changes made to the risk
management plan, we will hold staff meetings twice a year, to inform them of the new
policies and regulations.
