Humble submission before your office that I Maguni Prasad Singhsamanta
S/o- Budhanath Singhsamanta alias Budhanath Pradhan resident of village-
Uparsahi, Po-Prataprudrapur, PS-Balipatna, Dist-Khurda Odisha do hereby
submit that I had changed my name from Maguni Pradhan(Old Name) to
Maguni Prasad Singhsamanta(New Name) by swearing an affidavit before
the Executive Magistrate, Cuttack vide no: 4981 on dt: 13-04-2017 and it is
required to be published in the Extraordinary issue of Odisha Gazette so that
it shall be accepted as such at all places.

Therefore, my application is before your kind self for grant of permission for
Publication of Notice in Extraordinary issue of Odisha Gazette, for which I
shall remain ever obliged to you.
