General Release and Waiver of Liability

AS A CONDITION FOR MY PARTICIPATION IN A DIY STUDIO CLASS WITH

WAREHOUSE 13, I AGREE TO ASSUME ALL RISKS OF SUCH PARTICIPATION BY
SIGNING THIS GENERAL RELEASE AND WAIVER OF LIABILITY. I UNDERSTAND THIS
AGREEMENT AFFECTS MY LEGAL RIGHTS TO RECOVER DAMAGES SHOULD I BE HARMED
OR INJURED. I HAVE READ IT CAREFULLY AND MY SIGNATURE INDICATES THAT I
HAVE READ AND UNDERSTOOD ITS TERMS.

1. Release and Waiver. I hereby release Harper & Co. LLC, d/b/a Warehouse 13, its
principals and any employees, from all liability relating to injuries that may occur during a DIY
class at ________________. By signing this agreement, I, for myself and my heirs, agree to
hold Harper & Co. LLC, d/b/a Warehouse 13 entirely free from any liability, including financial
responsibility for injuries incurred, regardless of whether injuries are caused by the
negligence of Harper & Co. LLC, d/b/a Warehouse 13. I will obey safety precautions as listed
in writing and as explained to me verbally. I will ask for clarification when needed. I have read
and fully understand the above important information, the warning of risks below, assumption
of those risks and waiver and release of all claims.

2. Assumption of Risk. I acknowledge the risks involved in participating in a class

involving the use of power equipment including sanders and drills. I am participating voluntarily,
and all risks have been made clear to me. Additionally, I do not have any conditions that will
increase my likelihood of experiencing injuries while engaging in this activity.

3. Alcohol Consumption. Participants in Warehouse 13 classes are permitted to bring

their own wine and beer for responsible consumption during the class. If I am to consume
alcohol, I hereby swear and affirm that I am over the age of 21 and will drink responsibly
and do so at my own risk. I have designated a driver or will call Uber or a taxi in the event that
I am too impaired by the consumption of alcohol to drive.

4. Photo Release. I acknowledge that during today's class, my photo and or video may be
taken and I hereby release such photos/videos for use in marketing and advertising for
Warehouse 13, including, print, website and social media.

5. Miscellaneous. If any part of this General Release and Waiver of Liability is for any
reason declared invalid, such will not affect the validity of any remaining portion, which will
remain in full force and effect.

By Signing below I fully understand and agree to the above terms, and I forfeit all right to
bring a suit against Harper & Co. LLC, d/b/a Warehouse 13 for any reason. I also agree that I
will not use any items created here for promotion of any other business that operates in the
same capacity as Warehouse 13.
