We are hopeful that the information we have enclosed with this letter, which has been approved by the College Consortium, will help you with a clear sense of how this field experience works. When the student has completed the required observation hours, please complete and sign the “FIELD OBSERVATION TIME LOG” and “FIELD OBSERVATION STUDENT EVALUATION” (with the student). These two elements should be returned to the student who will submit it to the instructor to record and sign the form for the student’s portfolio.
