Incident Report:

! I chose to do the University of Illinois campus recreation form because it was the
easiest to follow and was the most straight forward. I chose this one because when the
next person to see it would be the manager and it was the form I believed would be the
easiest for me to get the message across to my manager. It had all the information
needed in the best format I thought out of the 3. The other 2 forms had a ton of
information that I felt was necessary for an accident like this. The ambulance was never
called and the young male was picked up by his parents. Although the parents said he
was ok we still needed to do our due diligence and follow up and write up a report about
the incident. The kid was aware of all his surroundings after he hit his head and no
blood was drawn. We were able to contact his parents very quickly and they
immediately came to pick him up. The young male was able to answer all the questions
at the end of the report.
