I. Grammar/Spelling
Leone is a very cooperative and obedient student who is eager to learn and share with his
teachers and peers. He is motivated to learn and works hard. Leone responds well to positive
reinforcement and feedback and is noticeably happy when complimented on his work.
Leone shows significant difficulty in grammar. He is able to write sentences if he is told them
aloud or word by word, but he often misspells words and forgets to add punctuation marks. If he
is unsure how to spell a word, he will stop until he is assisted with sounding it out. Leone often
forgets to capitalize the first letter of his sentence and will not do so without prompting. He will
sometimes randomly capitalize letters in his writing. When asked to write two sentences, he
wrote them but without capitalization and punctuation. He also displays difficulty with spelling.
He tends to spell words phonetically, especially if they are unfamiliar words. He often leaves out
the silent e at the end of words. When asked to find spelling and grammatical errors in his own
writing, Leone is unable to do so. He is also unable to find the errors in other words, such as
bak and swimng. Leone did not correct any grammar. If prompted to show what a sentence
ends with, only then will Leone add a period to the end of a sentence.
Leone is very excited when speaking about his personal life, more specifically his family and
swimming. When answering questions about his own life and things that interest him, Leone
responds with confidence and clarity. However, he struggles to keep eye contact consistently
when speaking and does not respond in full sentences. He also has difficulty pronouncing all
words clearly. When asked to write a personal narrative or letter, Leone struggles to write with
details and transition words. For example, I went to the beach. I had fun. We went home.
Leone is unable to write details or tell a sequence of events without guidance and support. He
also struggles to retell events and recall details without leading questions. When reading a text,
Leone is unable to answer comprehension questions that follow. He fails to write in complete
sentences and will normally sit quietly without asking for help. However, if read a story aloud,
Leone will accurately recall details from the story and proceed to write answers to questions with
guidance. Leone struggles to answer multiple questions at a time, but will complete the
questions if guided to answer them one at a time.
III. Math
Leone is very enthusiastic about math and participates most during this subject. He is easily able
to identify hundreds, tens, and ones within a three-digit number. However, he struggles to write
both addition and subtraction sentences based on word problems. Unlike other subject areas,
Leone is very confident in math and is much more eager and enthusiastic about completing math
work and participating.
Leone struggles significantly in explaining the reasoning behind the math steps he completes.
When asked to explain how he solves a math problem, he looks back at his work but has
difficulty making sense of it and explaining it in words. His confidence noticeably weakens, as
he has to write, even in math. He is unable to verbalize or write down all the steps necessary to
solve a problem (even one that he solved correctly) and struggles to even write down one step.
He is only able to solve mathematical problems and struggles with word problems, explaining
his answer, and writing down the steps he used to solve the problem.
Leone enjoys listening to stories and being read to. He performs better academically when he is
read a text rather than reading it on his own. When in a large group, he tends to lose focus and
stares at the floor or wall. However, when he is in a small group or being read a story one on
one, Leone is able to focus significantly better. Leone is not able to accurately retell a story that
he has read on his own, even with guiding questions. When retelling a story, he fails to use any
transition words. However, when a story is read and Leone is asked literal questions throughout
the story, he is able to answer them with confidence and accuracy. He, however, fails to answer
questions in full sentences. He takes long pauses to answer questions but it is clear that he is
thinking about how to answer.
PART C: GOALS AND OBJECTIVES
Long Term Goal #1: Leone will be able to write a paragraph using correct capitalization,
spelling, and punctuation.
Long Term Goal #2: Leone will develop his comprehension and use of story grammar-
character, setting, problem, solution- in order to organize thoughts and express sequences of
events in grade level passages/stories.
Long Term Goal #3: Leone will solve two and three digit addition and subtraction word
problems with grouping and regrouping.
Long Term Goal #4: Leone will ask and answer questions about key details in a text with
prompting and support.
