The patient is awake, conscious and coherent. The patient is in distress. Vital signs were taken
and monitored accordingly. The patient’s vital signs were: blood pressure of 140/80, pulse rate
of 78 beats per minute, respiratory rate of 20 cycles per minute, and temperature of 36.5. The
patient vomited 3x and complained headache and nausea. The doctor ordered ondansetron 8
mg IM to address nausea and vomiting and amlodipine to address high blood pressure.

degree celcius. Vital signs were taken and monitored accordingly. The patient’s vital signs were:
blood pressure of 120/80, pulse rate of 89 beats per minute, respiratory rate of 21 cycle per
minute. The doctor ordered paracetamol to address the fever of the patient.
