Incident & Injury 1: Prevention:

 Worked at pizzeria  Utilize proper tools
 Attempted to clean dough on roller by hand  Turn off the machine first
 Lost 3 fingers  Request assistance from co-workers

Incident & Injury 2: Prevention:

 Cleaning machines on a rainy day  Follow proper safety instructions
 Jumped off from 6 feet after cleaning  Clean it with co-worker
 Lost a leg as it got sucked into machine  Use a ladder to get down
