I. INFORMANT
a. First Informant

Name: Lady Tsunade

Address: Davao City

Relation to Patient: Elder Sister

Length of Time Known to Patient: from the time the patient was born

Apparent Understanding of Present Illness of Patient:

Lady Tsunade understands the patient’s illness as curable. She wanted to provide
wellness to his brother by letting him stay at the mental hospital for quite a long time.
Actually, she really did not expected that this would happen to his brother since he was
a good, generous and responsible person.

Other Characteristics and Attitude of Informant:

Lady Tsunade showed willingness to cooperate and answer questions. She had good eye
contact, and it was obvious that she was telling the truth about the condition of his
brother.

b. Second Informant

Name: Shikamaru

Address: Davao City

Relation to Patient: Friend/Brother-in-law of Lady Tsunade

Length of Time Known to Patient: since the year of 2000

Apparent Understanding of Present Illness of Patient:

Shikamaru believed that, Naruto’s condition will be worsening if Naruto don’t want to
cooperate, especially in taking medications. Hence, Naruto once stop taking his
medications in the past treatment.

Other Characteristics and Attitude of Informant:

 During the interview, Shikamaru couldn’t maintain eye to eye contact, talks softly and
couldn’t direct answer when asked and shows sympathy to Uzumaki Naruto.

c. Third Informant

Name: Hatake Kakashi

Address: Davao City

Relation to Patient: Brother-in-law

Length of Time Known to Patient: since Uzumaki Naruto was in Grade 3

Apparent Understanding of Present Illness of Patient:

The informant was surprised with the first diagnosis of Uzumaki Naruto. He didn’t
expect it to happen since Naruto was a good and responsible person. He believed that
the client’s condition comes back after several months or years was because Naruto do
not have strong relationship with his brothers since they have different vices. Also,
Naruto do not have a good relationship with his twin because Naruto do not want to be
defeated.
The informant thinks also that this misunderstanding might be one of the reasons why
Narutos’ condition worsens. Also Naruto has plans in his life. Failures of some of these
plans might contribute to his depression.

Other Characteristics and Attitude of Informant:

Hatake Kakashi when interviewed answers directly to what has been asked, he talks
softly, he had a good eye contact and looks sympathized for the condition of Uzumaki
Naruto.
