ALCOHOL TOLERANCE TEST EVALUATION

TO WHOM IT MAY CONCERN:

This is to certify that MS. CHRISTY REYES, 20 years old, with residence at Aurora Hill, Baguio
City, Philippines, was examined and treated in this hospital on December 22, 2014 with the following
findings:

Findings:

1) Possesed ALDH2/2 gene, an alcohol-rejecting gene

2) Physical contours show fatigue
3) BAC (g/100 ml of bloodor g/210 l of breath) equates 0.25 - 0.40

Remarks:

*Alcohol tolerance lower than the usual ALDH1/2 gene that is a major "alcoholic" gene.

*Fatigue influenced tolerance

* 0.25 - 0.40 BAC results to stupor

*stupor normally results to General inertia; approaching loss of motor functions; Markedly
decreased response to stimuli; Marked muscular incoordination; inability to stand or walk;
Vomiting; incontinence; Impaired consciousness; sleep or stupor
