AFFIDAVIT OF DISCREPANCY

I, ARNEL M. TELAN, Filipino, of legal age, married and resident of 146

Taguinod St., Linao Norte, Tuguegarao City, Cagayan, after having been
duly sworn to in accordance with law, do hereby deposes and states
THAT:

1. I am married to NORA T. TELAN, our marriage having been

solemnized by the late parish priest of Rev. Fr. Clement Daelman on
May 8, 1993;

2. Our marriage was duly registered at the Office of the Local Civil
Registrar of Tuguegarao City, Cagayan;

3. In all of my records and transactions, I have been using and is

known for the name ARNEL M. TELAN;

4. However, when I secured our Marriage Certificate at the Office of

the Local Civil Registrar Tuguegarao City, I discovered that my
name as indicated therein was ARNEL ARNOLD TELAN instead of
ARNEL M. TELAN as appearing in my birth certificate issued by
the NSO and in all my other records;

5. I hereby affirm and confirm that the names ARNEL ARNOLD

TELAN and ARNEL M. TELAN refer to one and the same person;

6. I have executed this affidavit to attest to the veracity and

truthfulness of the foregoing statements and to facilitate the
correction of my first name in our marriage certificate with the
Office of the Local Civil Registrar of Tuguegarao City, Cagayan.
