Due to the conflict of PHS's Retreat (March 2 to 3), involving Grade 10 and Grade 12 students, and the
Basketball Game of PHS Basketball Team against Philippine Institute of Quezon City (March 3 at 10AM), I
acknowledge that I'm allowing my son ____________________________________ from Grade ___ to
leave the Retreat earlier at 6 o'clock in the morning on the 3rd day of March 2018 to participate and
compete in the Alliance Basketball League to represent PHS which is held in Philippine Chen Kuang High
School.

I'm also acknowledging that I'm allowing my son to be fetched by Mr. Lepsin Tan (Father of Mark Lenard
Tan, Grade 12) and to ride on his car going to the playing venue.

I appreciate your support and understanding.
