assessed safety practices and laboratory environment, these include but are not limited to.

Laboratory weekly safety sweeps have been performed __________

Environmental Surveillance Rounds in Lab have been performed

And corrective action taken for any deficiencies found. __________

Safety inservices provided to staff and have been completed. __________

Effectiveness of Chemical Hygiene Plan has been done/ reviewed. __________

Effectiveness of Work Practice/Engineering Controls is complete. __________

Fire Drills have been taken place in the laboratory. __________

Chemical Inventory is current. __________

Noise Testing has been performed. __________

Laboratory Safety Policies and Procedures have been reviewed. __________

Accident Investigations have been completed and corrective

action taken as appropriate. __________
