Care Plan for Vulnerable Patient

Due to Dr. Cheever jcheever@roseman.edu No later than midnight 3/21/18

You will create a care plan for the patient. This involves a medical, dental and
social history with recommendations to improve the patient’s quality of life,
physical health and dental health. Do not get stuck in a tooth by tooth
treatment. Look at the global issues.

Case:

Patient: Gary Joseph, 65 years old. Gary is a Vietnam Veteran and has been
homeless for eight years. He arrived in the dental clinic and was assigned to you
today. He walks with a limp and is in the office with pain in tooth #2 with pain
duration of three days. Vital signs 134/84, Pulse 70. He has a medication list and
says he take them when he is able to obtain them. He is now in a long term
veteran’s home. Med List: Sertraline 100mg qd, Clonazepam 2mg q.d., Labelatol
200mg b.i.d. and Betamethesone Ointment as needed. Gary smokes tobacco
when he can get a cigarette. Oral Findings: Heavy Calculus throughout his mouth,
mobile teeth and bleeding gums. He only has twelve teeth-all Class III mobile. He
was an IV drug user in the past. Most of all he wishes to get out of pain.

Develop histories and then a proper care plan for Gary. Do this on your own. Put
your name on the paper if you hand it in to my office or email it to me when you
are done. If I don’t get an assignment, you will be marked as absent. Tell your
friends.
