been duly sworn, depose and say:
That I, RUBEN R. PADILLA, married and of legal age,
currently working as an analyst at the Palawan Diagnostic
Laboratory, Puerto Princesa City, Palawan.
That I personally know that the person named herein in
the complaint and the person whom I personally know are one
and the same person.That he is my classmate in High school,
