Psychiatric History
a. Occupation: 3rd year student nurse right now but back then, when I was in high school vacation I tried
on how to make a charcoal with my cousins in the province to have an extra allowance.
b. Psychosexual: Currently in a relationship but no label.
c. Family: Both Parents are separated but they are sensitive to our needs.
d. Military record: Not Applicable
e. Social adjustment: I’m shy at first but I do the stupidest random things when I get comfortable with
someone.
f. Habits: I drink alcohol occasionally, and both sleeping and eating is better after all work is done.
g. Interests: I like watching and waiting for new episodes every Sunday of my favorite anime, listening to
music and playing basketball to relieve my stress.
h. Environment: Adapt quickly to a new environment since my father is a police and always assigned to
different place so we need to go with him.
i. Personality and character structure: Easy to forgive and does not want seeing someone fighting.
j: Strengths of client: My mother is my strength and motivation to fulfill my dreams in my life, at the
same time my weakness.
