Survey Activity

You are to write a survey of at least 10 questions, of at least 5 types

(covering demographic and content issues).
• Make sure you include a brief introduction making the reader aware
of approximately how many questions are included and how long it
will take to complete the survey.
• Keep in mind, if this were a survey being used for research, you
would need a cover letter approved by the IRB.

Your survey may address any aspect of radiation oncology, patient care, or
medical dosimetry education.
• Ensure the questions are collecting data that can be analyzed easily
and give you the answers you need (to answer research questions
you might have).
• Open-ended questions are generally not recommended because
data collection is too time consuming, unorganized, and
inconsistent.

It may be designed as appropriate for either experimental or naturalistic

research.

The Qualtrics Survey tool will be used for this assignment. When the survey
is completed, make sure your survey is active, and then copy and paste
the link to the discussion forum for peer review.

It will be graded on the appropriate use of instructions and questions as well

as on the clarity of the questions and their organizational structure.
