Weekly Diary for Emily Sinn

Week of: March 19th, 2018 – March 23rd, 2018

Date: 3/19 Total hours: 8 am – 4 pm (8 hours)


 ICU rounds on 8 new patients
 Provide tube feed adjustment recommendations to nursing and Physician during rounds
 Consult with telemetry unit patients and provide low sodium diet education to a CHF
patient
 Chart on all patients
 Diabetes Management class from 2 – 4 pm

Date: 3/20 Total hours: 8 am – 4 pm (8 hours)


 ICU rounds on 1 new patient and 2 follow-ups; initiate supplement for malnourished
patient in the ICU
 Visit telemetry unit patients, provide nutrition education regarding a low sodium diet,
CHO controlled diet and a sodium and potassium restricted diet for renal disease
 Chart on all patients, complete nutrition education assessments and initiate supplement
recommendations
 Work ahead and visit/chart on patients scheduled to be consulted with tomorrow

Date:3/21 Total hours: 6 am – 2 pm (8 hours)


 In early out early due to snow storm
 No ICU rounds due to low patient census
 Chart on 5 new patients
 Consult with patients; 1 malnutrition screening and provide nutrition education regarding
a low fiber diet
 Chart on all patients and review for co signature with Dietitian

Date:3/22 Total hours: 8 am – 4 pm (8 hours)


 Chart on patients in the ICU
 Observe and assist in TPN initiation, calculations and recommendation form
 Consult with 3 patients in telemetry units
 Work ahead for tomorrow and consult with several follow-up patients to check their
knowledge following nutrition education and continue to monitor their intake
 Complete charting for all patients and initiate supplement
 Use extra time to review EatRightPrep information
