AFFIDAVIT OF LOSS

KNOW ALL MEN BY THESE PRESENTS:

I, Jake Walang-Kwenta, of legal age, Filipino, single, and a resident of Marigold Street
Lakeview Homes Putatan Muntinlupa City, after having been sworn in accordance
with law, hereby depose and say:

1. I am the holder of a school identification card issued by San Beda College

Alabang;

2. That I used to put said identification card inside my wallet;

3. That sometime on January 13, 2018, when I arrived home from school, I
discovered that my school ID card was no longer in my wallet where I used to place it;

4. That despite diligent search and efforts to locate the said identification card, I
could not find the same such that I now believe that said school ID card is now lost
beyond recovery;

5. That said ID card was not confiscated by the authorities or used in violation of
the law;

6. That I am executing this affidavit to attest to the truth of the foregoing facts and
to request for a replacement of said school ID card from San Beda College Alabang.

IN WITNESS WHEREOF, I have hereunto set affixed my signature this 30th day of
January 2018 at Muntinlupa City.

this 30th day of January, 2018. The affiant, whom I identified through the following
competent evidence of identity: PRC ID card valid from 7/5/2016 to 12/2/2019 issued
by Professional Regulation Commission, personally signed the foregoing instrument
before me and avowed under penalty of law to the whole truth of the contents of said
instrument.
