ASSIGNMENT 1

I. THIS ASSIGNMENT TO BE DONE IN GROUP OF TWO (2).

II. WRITE A REPORT ON ONE THE MAJOR WORLD ACCIDENT IN OIL AND GAS
OR PETROCHEMICALS INDUSTRY AND THE REPORT MUST CONSIST OF THE
FOLLOWING:

a. Accident Description
- Identify the causes of the Accident
- Emergency action taken during the accident.
- Construct Fault Tree Analysis (FTA) for that particular accident.
b. Onsite Impact
- Plant Equipments and Employees (Injuries, Death etc)

c. Offsite Impact
-Surrounding Impact (Environment, Health etc)

d. Prevention action and Control taken in avoiding the accident happen again.
- Management
- Technical

METHOD OF ASSESSMENT:

REPORT:

a. Report consist of Introduction, discussion, conclusion and references.

b. Each group must submit by 15th September 2017 .
c. Late submission will not be accepted.
d. Assessment (mark) on group work basis.
