Prolog:

When we talk about patient safety, we are really talking about how hospitals and other health care
organizations protect their patients from errors, injuries, accidents, and infections. While many hospitals
are good at keeping their patients safe, some hospitals aren’t. Patient safety was defined by IOM as the
prevention of harm to patients. Patient safety has been defined as those that reduce the risk of adverse
events related to exposure medical care across a range of diagnoses or condition. There are 6
international patient safety goals (IPSG):

1. Identify patient correctly


2. Improve effective communication
3. Improve the safety of high-alert medications
4. Ensure safe surgery
5. Reduce the risk of health-care associated infections
6. Reduce the risk of patient harm resulting from falls
