    Documentation for Discipline Interventions

    Directions: On first infraction, fill out all information and hand it back to me. This will serve as
    documentation if you ever need to be sent to the office.

    On the 10th and subsequent infractions, you will be sent to the Grade Assistant Principal.
    They will then handle this chronic behavior problem.

    Note to Student: It does not “start over” at this point. This discipline problem will continue to
    be handled by administration for EVERY infraction which means you will be missing a lot of class
    time and be in danger of failing the class.
