    Applying for Housing – Final Checklist

    Proof of Identity
    Please ensure you have provided 3 different types of identification:
     Make sure one of your proof of identity documents is from the Primary ID section and
    includes your photograph and signature.
     Another two documents can come from Primary ID or Secondary ID. You can only use a
    document once.

    Primary ID / Secondary ID /


    Drivers licence Medicare card
    Passport Pension card
    Key pass Centrelink concession card
    Birth Certificate (or extract) Electoral Role Registration
    Citizenship Documents Utility Bill

    All documents must be current. Expired documents will not be accepted.

    Proof of Income
    For all household members over 18 ensure you have provided 2 months of current bank
    statements and at least one of the following:

    Proof of Income /


    Most recent Centrelink Income Statement (no older than 28 days)
    13 consecutive weeks of payslips
    Current tax return statement (if self employed)
    Profit and loss statement (if self employed)
