อุบัตกิ ารณ์และสาเหตุของโลหิตจาง
อุบัติการณ์การพบภาวะโลหิตจางระหว่างการตัง้ ครรภ์ขึ้นกับปริมาณเหล็กในร่างกายที่มีอยูเ่ ดิม และจากการ
รับประทานอาหารตัง้ แต่ก่อนการตั้งครรภ์(4)และพบว่าหญิงตั้งครรภ์ที่มีระดับ Hb ปกติมาตั้งแต่ก่อนตั้งครรภ์จะเกิดโลหิต
จางจากการเสียเลือดในระหว่างการคลอดได้ถึงร้อยละ 20 และในประเทศไทยพบอุบัตกิ ารณ์การเกิดโลหิตจางในสตรี
ตั้งครรภ์ร้อยละ20-30 สาเหตุสาคัญที่ทาให้เกิดโลหิตจางแสดงในตารางที่ 1

ผลของโลหิตจางทีม่ ีต่อการตั้งครรภ์(5-7)
1. เพิ่มความเสี่ยงต่อการเกิดการคลอดก่อนกาหนด
2. เพิ่มความเสี่ยงทารกรกเกิดน้าหนักตัวน้อย
3. มีผลต่อการสร้างหลอดเลือดใหม่ในรกในระยะแรกของการตั้งครรภ์
4. เพิ่มโอกาสการเกิดภาวะแทรกซ้อนปริกาเนิด
4. Megaloblastic Anemia
เป็นโรคที่มีความรุนแรงสาเหตุเกิดจากความผิดปกติของเซลล์ต้นกาเนิดในการสร้างเม็ดเลือด(hemopoietic stem
cell) อาการจะเป็น hemolytic anemia แบบเรื้อรัง ความรุนแรงมีตั้งแต่น้อยจนถึงเสียชีวติ ตัวโรคอาจได้รับการกระตุ้นจาก
การให้เลือด และการผ่าตัด ร้อยละ 40 ของผู้ป่วยจะมีอาการของเส้นเลือดดาอุดตัน ไตวาย ความดันโลหิตสูงมีอตั รา
มารดาเสียชีวิตถึงร้อยละ 10-20 โดยร้อยละ 50 มักเกิดขึ้นหลังคลอดสาเหตุเกิดการอุดตันในเส้นเลือดดา การรักษาทาด้วย
เปลี่ยนถ่ายไขกระดูกเท่านั้น และการป้องกันการอุดตันของหลอดเลือดดาโดยการให้prophylactic anticoagulant(10)
