Below is a sample writeup of a patient without any significant physical exam findings.
Please pretend as though you saw one of disease cases from the handout given in class &
replace the physical exam findings below with those listed in the case. If no information
was given in the case, assume a normal finding (i.e. such as a finding from your lab

HEENT
Head: The skull is normal cephalic, atraumatic (NC/AT). Hair with average texture. No
head, sinus, or TMJ tenderness.
Eyes: Sclera white, conjunctiva pink. Pupils are 4 mm constricting to 2mm, equally
round and reactive to light and accommodations (PERRLA). Red reflex present. Extra
ocular muscle function is intact (EOMI). Symmetrical corneal light reflex. Visual fields
full to confrontation. Visual acuity 20/20 bilaterally. Upon fundoscopic exam, disc
margins sharp; no hemorrhages or exudates, no arteriolar narrowing. [Please record only
what you were able to see with ophthalmoscope. If not visualized, state ‘not visualized’.]
Ears: Pinna, tragus, and external canal nontender. Ear canal clear and tympanic
membranes (TMs) with good cone of light. Acuity good to whispered voice. Weber
midline. AC > BC (Rinne) in both ears.
Nose: Nasal mucosa pink, septum midline. Nares patent bilaterally. Olfaction intact.
Throat/Mouth: Oral mucosa pink, dentition good, pharynx without exudates. Tonsils
symmetric, no adenopathy. No buccal nodules or lesions.
