Dear Sir/Mam,
It is regarding the issuance of MSD drugs to my wife as she is suffering from
kidney disease and as undergoing a medical treatment from RML hospital where doctor has
advised for peritoneal dialysis. I am govt employee and also the CGHS card holder.

Request you to please issue the MSD drugs as per the doctor advised.

My CGHS card number is 2686257 and my wife Uma rani card number is 2686260.
