INFORMED CONSENT

Investigators:

“We are interested in learning more about the Causes of teenage smoking among the
senior high school students of Saint Paul University Philippines. You will be ask to answer
an assessment tool rgarding the study. This will take approximately 10-15 minutes of
your time. All information will be kept anonymous. This means that your name will not
appear anywhere and no one except me will know about your specific answer. “

“The benefit of this research is that you will be helping us to understand the causes of
teenage smoking among Grade 11 and 12 students of Saint Paul University Philippines.
This information shold help us to bring enlightenment on the topic of our interest. The
risks to you for participating in this study is that there might be a disclosure of
information to other persons other than the researchers. This risk will be minimized by
strictly maintaining privacy and confidentiality as well as storing the collected data
securely. If you do not wish to continue, you have the right to withdraw from the study,
without penalty, at anytime. “
