INSTRUCTIONS
Write the names of four people in the empty boxes at the top of the questionnaire.
Ask four people the questions on the left hand side of the Questionnaire and record a short note of their answer in the correct box.
Practise answering the questions as fully as you can.
Now look at the results as a group and use the example questions to discuss the data that you obtained.
